Commitment is a two-way street

I am committed to guiding you toward healing and wellness. You will need to commit to making all appointments and doing all assigned exercises as directed so that you can achieve your best results!

Follow these simple steps to begin your journey towards better health and wellness.

1

First, download the Discovery Paperwork & Photo List. Take your time & make sure to fill out each page completely! Each page provides clues that when put together, help me uncover the root(s) of your symptoms. Take photos of yourself as described in the Photo List. Send the completed Discovery Paperwork and photos to [email protected].

2

Next, book & pay for your virtual Complete Assessment.

3

Then, attend your 90 minute Complete Assessment where we will give you a brief introduction to Myofunctional Therapy, thoroughly review all of your paperwork & photos, and discuss your goals for therapy. Together, we will evaluate your commitment to therapy & positive outcomes as well as discuss a plan for moving forward. After the assessment, I will send you a comprehensive written report within 3-5 business days.

4

After that, I will extend an offer to participate in an appropriate therapy package with me. To accept my offer for a spot in therapy, simply sign a consent form & pay for the therapy package. After I receive consent and payment, I will create a personalized therapy plan with your goals & needs in mind. I will also mail a therapy supply kit to your home.

5

After you receive your kit, take the rest of the photos from the photo list and email to [email protected]. Finally, schedule your first 30 minute therapy session.
